Tutoring Center Registration Application
Tutoring Center Registration Documents: (Two copies):
- This registration application form
- Registration application form
- Weekly class schedule
- Licenses for building use as a tutoring center, copies of approval documents, fire safety equipment inspection documents; if the building has interior renovations, interior renovation qualification certification must be attached.
- Blueprint of the classroom (must indicate classroom area (in square meters), office, and safety and health facilities).
- Qualifications (i.e., diplomas) and identification documents of the founder and principal; if it is a vocational tutoring center, additional certification related to the principal's skills must be attached. (If a company is establishing a tutoring center, a company registration certificate, company registration form or transcript, and pre-application documents for the company's business registration must also be attached.)
- If there are two or more founders, a notarized partnership agreement must be attached, designating one person as the representative, along with a list of co-founders.
- List of proposed teaching staff, qualifications, and identification copies.
- A declaration that the establishment fund will not be used.
- A declaration from the founder, principal, and teaching staff regarding their employment.
- Inventory of assets.
- Proof of classroom usage rights (if leasing, the lease term must be more than two years).
- Property registration transcript or copies of ownership documents or tax certificates.
Note: Proof of establishment fund deposit (must be a fixed deposit for two years or more) must be submitted within 21 days after registration; the fund amount details are as specified in the "Short-term Tutoring Center Establishment and Management Regulations" appendix.
